Glucopuncture for Treatment of Fascial Pain in the Thumb. A Clinical CaseChronic pain is commonly managed conservatively, but there is growing interest in the application of regional injections in the superficial fascia with dextrose 5% in water (D5W). In many cases, the origin of vague musculoskeletal (MSK) pain originates from small lesions in the superficial fascia which are not visible on X-rays, Ultrasound or MRI. This article describes a case of chronic referred pain which was treated with a series of weekly D5W injections in the superficial fascia. This case is an invitation to design more clinical research to confirm whether this novel approach may become one of the new tools for patients suffering from vague MSK pain.

EMULGEL: AN EMERGING APPROACH TOWARDS EFFECTIVE TOPICAL DRUG DELIVERYDosage forms for topical drug delivery systems are applied directly to the skin in order to treat a variety of illnesses. The most recent and widely used drug delivery technology for hydrophobic medications is emulgels. Different polymers are used to prepare emulgel, which serves as both an emulsifying and thickening agent. The gelling capacity reduces surface tension and interfacial tension, increasing the viscosity of the aqueous phase and producing stable emulsions and creams. Emulgels offer significant advantages over both traditional and new vesicular systems when taking into account a number of factors. This formulation, which combines gel and emulsion, is thought to be a novel kind of drug delivery method. Emulgel is a highly promising product for use in dermatology since it is translucent, has an exquisite appearance, is greaseless, readily removed, easily spreadable, emollient, non-staining, and has a longer shelf life. It also has less potential to cause major adverse effects.

A REVIEW OF NATURAL GREEN ALTERNATIVES TO PSORIASIS TREATMENTA chronic skin condition called psoriasis causes patches of skin to thicken and become covered in silvery scales. Though less successful, topical chemical treatments are used to treat psoriasis; nevertheless, medicinal plants can offer an alternative. One significant and distinctive source of medications is medicinal plants. The three most often used herbs for psoriasis are neem, turmeric, and aloe Vera. The most typical signs and symptoms are small scaling areas, red patches on the skin, itching, burning, and pain; thicker, pitted, or ridged nails are also prevalent. Psoriasis is an inflammatory immune-mediated illness for which there is no long-term treatment. Although there are a number of ways to treat psoriasis, no single drug makes a complete and satisfactory recovery. Due to their accessibility and safety, herbal medications have the potential to be effective anti-psoriatic agents.

AN ANALYSIS OF DISEASE PROGRAM FINANCING IN NIGERIA USING THE NATIONAL HEALTH ACCOUNT DATA: DO GENERAL GOVERNMENT AND DONOR DISEASE EXPENDITURE CORRELATE?This study provides an in-depth analysis of domestic and external health financing situation in Nigeria by disease-specific conditions between 2010 to 2018. Key health financing agents in the country include households, firms, government, donors, and non-profit organizations. . The study adopted the descriptive analytic approach and the Pearson correlation analysis. It considers the correlation between donor expenditure per disease area and general government health expenditure. The study revealed that between 2010-2018, total expenditure by diseases was $72.92 billion. Further analysis by the core priority interventions shows that total expenditure on malaria was $29.0 billion, followed by non-communicable diseases (NCDs) $6.9 billion, while human immunodeficiency virus (HIV) treatment consumed $6.5 billion. Tuberculosis (TB) expenditure was $2.7 billion and Neglected Tropical Disease (NTDs) was $0.2 billion. The study found a strong correlation between aggregate donor expenditure and aggregate Federal Government (FG) expenditure (r=0.6), a medium correlation between the donor and State Governments (SG) expenditure (r=0.3), and a small correlation between donor and Local Government (LG) expenditure (r=0.2). However, disaggregating by disease intervention, the study showed that there is a small correlation between FG HIV and FG Reproductive Health (RH) expenditure and no correlation between FG malaria expenditure and donor malaria expenditure. On the contrary, the study finds a strong correlation between donor expenditure and (SG) HIV and SG RH expenditure and a small negative correlation between donor malaria expenditure and SG malaria expenditure. These findings indicate that while overall FG expenditure increases as donor increases financing, this increase may not be related to HIV, Malaria, and RH programming. However, donors are willing to work with state governments who are ready to put in more money for health especially for HIV and RH, and not necessarily for malaria. Thus, the study recommends that government should increase intervention/program targeting especially on Malaria as well as TB, and HIV.

GREEN SYNTHESISED SILVER NANOPARTICLE OF COLOCASIA ESCULENTA: INVITRO ANTIOXIDANT AND ANTIULCER ACTIVITYIn the present research, a methanolic extract of Colocasia esculenta leaves is used to produce silver nanoparticles in an environmentally friendly manner. The chemical families of flavonoids and phenols were highly present in this therapeutic plant. Once silver nitrate was transformed into silver nanoparticles (Ag NPs), Zeta potential, particle size analysis, and FT-IR were used to characterize the particles. Numerous functional groups surrounding Ag NPs were verified by the FT-IR investigation. It was discovered through particle size analysis that the synthesised silver nanoparticles were nano in size. Ag NPs' calculated zeta potential of -46.5 mV confirms particle repulsion and hence raises the stability of the formulation. Investigating Ag NPs' antioxidant and anti-ulcer qualities, the findings showed a strong capacity to scavenge free radicals and a notable boost in the extract's capacity to neutralize acids. It was discovered that the greenly synthesized silver nanoparticles were a potentially effective phytomedicine for ulcer treatment.

Sellar Reconstruction Post Endonasal Pituitary Surgeries: Case Series with Our ProtocolSella turcica is a bony depression situated on the intracranial surface of the sphenoid bone, and in order to prevent the postoperative leakage of cerebrospinal fluid (CSF) during transsphenoidal surgery, it is crucial to undergo complete sellar floor reconstruction. This study aimed to evaluate the effectiveness of endonasal reconstruction of the sella turcica to prevent CSF leakage. This study included a case series of (83) patients admitted to Kirkuk Teaching Hospital in northern Iraq, from the period (2015) to (2021), and they underwent endoscopic transsphenoidal approach by performing rescue flap bilaterally with preserving the blood supply in all cases. This was followed by evaluation of intraoperative cerebrospinal fluid leak according to Daniel F. Kelly classification of graded repair protocol with some of our necessary modifications to obtain the best outcome. The results confirmed that out of a total of 83 cases, the most common pathology for which surgery was performed was adenoma (62). In (49) patients, there was no leakage (grade 0) during surgery, (19) patients had (grade 1), (10) had (grade 2), and only (5) cases had (grade 3).In all cases, we reconstructed the saddle according to the grade of defect, type )1( repair was the most common followed by type )2.( Also, postoperative cerebrospinal fluid leakage was observed in only )4( patients. It was concluded that our modified protocol for intraoperative CSF leaks grading and the repair techniques designed using sellar reconstruction after endoscopic resection of the pituitary tumors have proven to be an effective and reliable approach for postoperative CSF leak prevention and controlling.

Management of a Giant Infected Renal Cyst in a Pregnant WomanA case report of 29-year-old pregnant woman with a giant infected renal cyst. Treatment involved antibiotics, tocolysis, percutaneous drainage, and sclerotherapy, leading to a reduction in cyst production flow. Follow-up after six months showed no residual cystic lesion.

“A Review of RCC Centred Radiomics in The Context of Benign And Malignant Renal masses”Due to radiologists' subjective interpretation, radiomics texture analysis offers objective image information that would not otherwise be available. In this paper I examined the implications of radiomics for the evaluation of kidney tumors and produced a thorough research. I looked up English-language research on radiomics applications in renal tumor assessment in the PubMed-MEDLINE database till 2023. 52 papers total are included in the analysis, which is divided into four categories: patient outcome prediction, nuclear grade prediction, renal mass differentiation, and gene expression-based molecular markers. The research's main goal is to accurately differentiate benign from malignant renal masses, with a particular emphasis on renal cell carcinoma (RCC) subtypes, angiomyolipoma without visible fat, and oncocytomas. Nuclear grade prediction may help in more appropriate patient selection for risk-stratified treatment. Individualized treatment regimens may be made easier by predicting patient responses and outcomes from targeted drugs, and gene mutations predicted by radiomics may serve as stand-in biomarkers for high-risk disorders. Studies have generally demonstrated that radiomics is better than expert radiological interpretation. Compared to subjective visual interpretation, radiomics provides a more objective way to diagnose kidney cancers. Adding more clinical and imaging data to radiomics algorithms will improve tailored therapy and boost the accuracy of tumor prediction.

Understanding Depression among Medical Students in Pakistan: Prevalence, Severity, and Implications for Mental Health SupportPurpose: This study aims to investigate the prevalence of depression among medical students in Pakistan using the Patient Health Questionnaire-9 (PHQ-9) scale, with a focus on understanding the distribution of depressive symptoms across different demographic groups.
Methodology: Data were collected from 1326 medical students enrolled in 110 medical colleges across Pakistan, spanning from the first year to the fifth year of study. Participants completed the PHQ-9 form online, allowing for random sampling. Depressive symptoms were categorized into minimal and mild (considered as no depression) and moderate, moderately severe, and severe (considered as depression). Statistical analysis was conducted to examine the prevalence of depression among participants, considering gender differences.
Findings: The study included 794 female participants and 532 male participants. Results indicate that 30.39% of participants experienced mild depression, 17.27% exhibited minimal depressive symptoms, 26.70% reported moderate depression, 14.71% displayed moderately severe depressive symptoms, and 10.94% showed severe depressive symptoms. Among participants, 52.34% exhibited no signs of depression, categorized as minimal and mild, while 47.66% displayed varying degrees of depression, including moderate, moderately severe, and severe.
Unique Contribution to Theory, Policy and Practice : By categorizing depressive symptoms and analyzing gender differences, this study provides nuanced insights into the prevalence and distribution of depression among medical students in Pakistan. These findings contribute to the existing literature on mental health in medical education, particularly in the context of South Asia. The study underscores the need for policy interventions aimed at addressing the mental health challenges faced by medical students in Pakistan. Implementing targeted support services and resilience-building programs within medical education institutions can help mitigate the impact of depression on student well-being and academic performance.

COMPLETE RESOLUTION OF LEFT RENAL CYST THROUGH HOMOEOPATHIC INTERVENTION: A CASE REPORTIntroduction: Renal cysts are mostly asymptomatic and found incidentally on imaging studies. Even though asymptomatic cases doesn’t require treatment, they can cause complications like enlargement, infection and sometimes progress to carcinoma which need surgical interventions. Spontaneous resolutions are rare.
Case summary: A 38-year-old man complained of pain in the right side of his upper abdomen. A left renal cyst, discovered incidentally on ultrasound, was identified with internal septations and measured 2.2*2.5 cm. The abdominal pain and cyst completely resolved within three months after administering the Homeopathic medicine Phosphorus 30c. This medicine was selected based on a comprehensive assessment of the patient's symptoms. The evidence was assessed using the USG report and MONARCH criteria.
Conclusion: This case report shows the effectiveness of homoeopathic intervention in renal cyst when selected based on the totality of symptoms and individuality. This case report can contribute to evidence-based studies in this topic and further studies are encouraged.

The production fo this encapulated biotecnological product imply the mixing of an acqueos phases (mRna ) with an lipidic phases (nanolipids). The graphene finded as reported, probably due by purification steps, because idrofilic it must to be encapsulated. The Raman spectra of registrated m RNA C19 vaccine not revealed graphene particle , but in literature it is reported that the direct tecchnique is no the best way to to detect well the nanolipids payload : so a negative results not imply this it not present inside the nanoparticle. Aim of this work is to investigate the manifacturing process of this biotechnological products submitting to the reseacrher an hypotesys of work than can explain all this pheneomena: the possible segregation of graphene inside the nanolipids. Great enphasys is given to the size , the chemico physical property like idrophilicity of the graphene particle versus the nanolipids used in some m RNA C19 vaccine, and the property of inside /outside this particles.

A RARE CASE OF SPONTANEOUS PERFORATION PERITONITIS DURING SECOND TRIMESTER PREGNANCY – A CASE REPORT AND LITERATURE REVIEW OF MANAGEMENT CHALLENGES OF DELAYED PRESENTATIONObjective – Bowel obstruction or perforation during pregnancy is a rare but life-threatening situation, and the management remains uncertain and patient dependant. We presented a case in our hospital with fever and acute abdomen in her second trimester, so we reviewed literatures and other cases to provide options for management based guidance to deal with this challenging situation.
Case Report – A case at gestational age 19 weeks who was diagnosed with delayed presentation of Intestinal Perforation was referred to our centre from a primary health care centre without any history of previous surgery and was managed surgically with Emergency Laparotomy. The patients WIDAL came positive, that along with low socio economic background supported our diagnosis of Typhoid causing the Intestinal perforation. She had a challenging post operative period the foetus was expelled in view of sepsis one week after the surgery. We researched pubmed and reviewed 10 English literatures to form a a conclusions enlisting management options and how to deal with delayed presentation- Al lesson learnt.
Conclusion – Our case presentation serves as a poignant reminder of the challenges faced by unusual presentations in acute abdomen during pregnancy. Timely intervention and a simultaneous cesarean section could be considered if the gestational age was large enough to avoid the comorbidities like maternal risk of sepsis that outweighs the risk of preterm birth.
