Low-flow priapism as a presenting feature of chronic myeloid leukemia: a 5-case experience and the importance of tyrosine kinase inhibitor therapyBackground: Low-flow priapism is a urological emergency that can cause permanent erectile dysfunction if not treated quickly. Blood diseases such as chronic myeloid leukemia (CML) can cause priapism, but it is very rare for priapism to be the first sign of CML, especially in young adults. Early diagnosis and treatment of the cause are essential to save erectile function and control the disease.
Objective: To describe the symptoms, treatment, and outcomes of five patients who presented with low-flow priapism as the first sign of CML, and to show the importance of using tyrosine kinase inhibitors (TKIs) in Sub-Saharan Africa.
Methods: This observational study was carried out between 2022 and 2025 in three tertiary hospitals in Sub-Saharan Africa. Five men aged 18–35 years came with painful, prolonged erections not related to sexual activity. Initial surgery (caverno-spongious shunts) did not fully solve the problem. Blood tests confirmed CML, and patients were given cytoreductive drugs followed by TKIs (imatinib).
Results: All five patients had very high white blood cell counts (500–644 G/L) and enlarged spleens. After starting TKI therapy, full detumescence occurred within 3–5 days, and erectile function was preserved. One patient who had paraplegia recovered fully within two months. No recurrence of priapism was noted during follow-up.
Conclusion: Low-flow priapism can rarely be the first symptom of CML in young adults. Surgery alone may not be enough; early treatment with TKIs is key for rapid recovery, preserving erectile function, and controlling the disease. This series highlights the need for quick diagnosis and combined treatment, especially in low-resource settings.

The Role of In Vivo Vibration Techniques in Understanding Tibial Bone Properties: A State-of-the-Art ReviewThe place of the in vivo vibration methods in the investigation of the mechanical characteristics of the tibial bone becomes a key to the development of the clinical diagnostics and research. Bone biomechanics, which is a combination of mechanical knowledge and biological knowledge, has been used majorly in the analysis of skeletal functionalities as well as the safety among the skeletal structures particularly the tibia which is a weight-bearing bone that is significant in the movement of humans. Conventional techniques, like X-rays and MRIs, will give the structural information, however these do not render the real life mechanical dynamics of bone. Vibration means The use of vibration in vivo What is the measurement of response to vibration of low frequencies to bone; these vibration methods have proven to be non-invasive accurate ways to measure bone health; particularly, bone density, stiffness, and elasticity. This review examines the use of the vibration measurement techniques on the tibia including osteoporosis, fractures and the post operation conditions. Through vibrational responses, these methods yield meaningful information with respect to the strength of the bone as far as injury prevention and the selection of rehabilitation measures are concerned. Due to emerging technologies like wearable sensors and computational modeling, the methods are also expected to improve the real-time measurements and accuracy to assist in clinically accurate decision-making in orthopedics.

Squamous cell carcinoma of the external ear: clinical and therapeutic aspects of a caseCarcinomas of the external auditory canal are a rare condition, accounting for approximately 0.2% of all tumors in the head and neck region. Their rarity explains the lack of reliable epidemiological data and the absence of consensus treatment recommendations. Diagnosis is based on imaging and histology, supplemented by the modified Pittsburgh classification, currently the most widely used method for assessing tumor spread. Management remains complex due to the lack of solid prognostic criteria and the diversity of possible therapeutic approaches. Surgery remains the standard option, although it is often invasive and associated with significant postoperative morbidity. Adjuvant radiotherapy is frequently offered, while the role of chemotherapy and exclusive chemoradiotherapy remains debated, particularly in locally advanced forms. Despite therapeutic advances, the prognosis for locally advanced forms remains poor.
The main objective of our study is to present, through this clinical case, the diagnostic and therapeutic characteristics of CEAC, as well as to identify the main prognostic factors associated with this rare disease.

Management of orbital complications of acute bacterial sinusitis: a case report and review of the literatureIntroduction: Orbital complications of acute rhinosinusitis (ARS) are the most common extra-sinus manifestations, particularly in children and immunocompromised patients.They represent a medico-surgical emergency due to the anatomical proximity between the paranasal sinuses and the orbit, which facilitates infectious spread.
Methods: We report the case of a 46-year-old female with poorly controlled type II diabetes who developed orbital cellulitis complicating an ipsilateral maxillary sinusitis. Clinical, radiological, and therapeutic aspects were analyzed and compared with data from the literature.
Results: The patient presented with stage II Chandler orbital cellulitis. Diagnosis was established through clinical evaluation and computed tomography. Management consisted of broad-spectrum intravenous antibiotics combined with surgical drainage, leading to a favorable outcome without sequelae.
Discussion: Orbital complications of ARS require early recognition and prompt intervention. The Chandler classification remains a valuable tool to stratify severity and guide treatment. Prognosis depends largely on early management, with blindness and intracranial extension being the most serious complications.
Conclusion: This case highlights the importance of rapid diagnosis and appropriate medico-surgical management of orbital complications in ARS to prevent vision-threatening and life-threatening outcomes.

Histopathologic Patterns of Triple-Negative Breast Cancer in Makurdi, North-Central NigeriaBackground: Triple-negative breast cancer (TNBC), defined by the absence of estrogen receptor (ER), progesterone receptor (PR), and HER2 expression, is an aggressive breast cancer subtype associated with poor prognosis and limited targeted therapy. TNBC prevalence is disproportionately high among women of African ancestry, yet regional histopathologic data remain limited in North-Central Nigeria.
Objective: To determine the histopathologic patterns and immunohistochemical profiles of TNBC in Makurdi, North-Central Nigeria.
Methods: A retrospective descriptive study of invasive ductal carcinoma (IDC) cases diagnosed between 2021–2024 at Benue State University Teaching Hospital. Only histologically confirmed TNBC cases with complete ER, PR, and HER2 status were included. Tumours were classified and graded using WHO 2019 criteria and Nottingham grading. Data on patient demographics, histologic subtype, and tumour grade were analysed using descriptive statistics.
Results: Among 290 IDC cases, 179 (61.6%) were TNBC. Patients’ ages ranged 20–89 years, with peak incidence in the 50–59 years’ group (32.1%). TNBC predominated in middle-aged women (40–59 years). Most tumours were high-grade invasive ductal carcinoma of no special type (NST), with a minority showing special histologic subtypes. Immunohistochemistry revealed ER negativity in 58.3%, PR negativity in 60.3%, and HER2 negativity in 66.2% of cases.
Conclusion: TNBC constitutes a substantial proportion of breast cancer in Makurdi, affecting predominantly middle-aged women, with high-grade invasive ductal carcinoma as the dominant histologic pattern. These findings underscore the need for region-specific diagnostic strategies, early detection programs, and tailored management protocols to address the high burden of TNBC in North-Central Nigeria.
