Understanding the Multifaceted Impact of Childhood Epilepsy: A Narrative Review

Background: Epilepsy is one of the most common neurological disorders in children, characterized by recurrent seizures that can significantly impact their physical, cognitive, and emotional well-being. While seizure control is a primary focus of treatment, growing evidence suggests that epilepsy affects motor function, cognitive abilities, and overall Quality of Life (QoL) beyond seizure activity itself. Many children with epilepsy experience mobility and balance impairments, increasing their risk of falls and limiting their ability to engage in physical activities. Additionally, cognitive deficits, such as memory loss, attention difficulties, and executive dysfunction, can lead to academic challenges and developmental delays. The use of antiepileptic drugs (AEDs), while essential for seizure management, may contribute to these impairments by affecting brain function and processing speed. Furthermore, epilepsy can have significant impact on their QoL causing psychosocial consequences, including social isolation, anxiety, and emotional distress, which impact both the affected children and their families. Given these widespread challenges, it is essential to understand the broader impact of epilepsy on other outcomes beyond seizure control. Objective: This study aims to examine the effects of epilepsy on balance, cognitive function, and QoL, highlighting key factors such as seizure frequency, AED use, and psychosocial challenges. Study Selection: This narrative review is conducted on databases from PubMed, Google Scholar and Research Gate. This review includes 10 relevant studies analysing the impact of epilepsy on balance, mobility, cognitive performance, and QoL. Conclusion: Children with epilepsy face significant challenges that extend beyond seizures, affecting their physical, cognitive, and emotional well-being. Motor impairments increase the risk of falls, cognitive deficits hinder academic performance, and psychosocial issues contribute to social difficulties and emotional distress.

A rare presentation of Uterine perforation after Two days of Dilatation and Curettage procedure in a Scarred uterus with Extruded Fetal parts in abdominal cavity- Case Report

A life threatening complication of intrauterine procedure (Dilatation and curettage)is uterine perforation besides having medico legal implications. Since medical abortion has been legalized, it has become very rare during the D&C procedure in recent times. Early diagnosis is key to survival, especially if an unsafe abortion has been performed. Therefore, if the patient has lower abdominal pain, and the history of surgical abortion may be unknown most of the time, a high index of suspicion should be maintained. This case report emphasizes an atypical presentation of uterine perforation in a 28 year old woman with H/o Caesarean section 1 year back, who underwent D&C procedure at 19 weeks of pregnancy. She was discharged after the surgery, assuming there were no complications. Two days after the procedure, patient presented with complaints of acute abdominal pain and distension associated with features of internal bleeding and Bowel obstruction. Trans abdominal Ultrasound was performed, uterine perforation with Massive Haemoperitoneum was diagnosed. X ray abdomen erect view confirmed Bowel obstruction. Intra operatively, Fetal parts were removed from Pouch of Doughlas and abdominal cavity. Prompt recovery was achieved after surgical intervention. This case report highlights the importance of increased awareness during the D&C procedure. Furthermore, it also emphasizes the importance of pre-operative counselling and post-operative monitoring to ensure early detection and prompt management of life-threatening complications.

Urinary Tract Infection Due to Trichosporon asahii in a Pediatric Patient with Nephrotic Syndrome: A Case Report

Trichosporon asahii is an emerging opportunistic fungal pathogen increasingly identified in immunocompromised hosts. We report a rare case of urinary tract infection (UTI) caused by T. asahii in a 6-year-old girl with nephrotic syndrome undergoing prolonged corticosteroid therapy. The diagnosis was confirmed at the Parasitology-Mycology Laboratory of the Mohammed VI University Hospital in Marrakech. This case highlights the diagnostic challenges, antifungal resistance patterns, and therapeutic considerations associated with this unusual infection in pediatric patients.

Oral Fibroepithelial Hyperplasia: Clinical Presentation and Management in a Rare Case

Fibromas represent the most prevalent type of fibrous overgrowths found in the oral cavity, usually arising as a result of prolonged, mild irritation. Management includes meticulous plaque control, removal of underlying causative factors, and conservative surgical removal to minimize the risk of recurrence. Hence, it is crucial for clinicians to possess a comprehensive knowledge of these lesions to effectively distinguish them from other lesions with similar clinical appearances. In this case report, a 60-year-old female patient presented with a gingival overgrowth on the left anterior maxilla, persisting for the past three months. The presence of ossifications within fibroepithelial hyperplasia in this age group is a rare and unusual finding, as noted in previous case reports. The lesion was surgically excised using electrocautery, and postoperative healing was uneventful within one week. Histopathological differential diagnoses of fibroepithelial hyperplasia include pyogenic granuloma, peripheral giant cell granuloma, and irritational fibroma. Electrocautery, a precise and efficient surgical modality, was utilized in this case for effective lesion removal with minimal tissue trauma and is preferred for its ability to minimize bleeding, reduce infection risk, and promote faster healing.
